That is something that only the machine shop you take it to would know. My advice is to call around to your local machine shops and get quotes, then see who will beat the others' prices. There is no set price on machine work, each shop will have a different hourly rate (though there MAY be places that specialize in engine work and they may have a set "menu" type pricing for decking blocks, boring and honing, etc.).

You would bore the cylinder, not the piston. For that oversize, you could probably get oversize rings, but really ought to fit oversize pistons as well. You may want to polish the cylinder head where the combustion chamber meets the cylinder to prevent hot spots.

4 thousandths is not that much. Why not ask a shop if you can go even larger and install larger pistons, provided there is enough meat on the block to handle it. They should not charge you for an estimate. Go to at least 3 shops for their quotes. And, ask for references.
