how to install the 08695-00370 y-adapter cable in 2001 prius?

Default how to install the 08695-00370 y-adapter cable in 2001 prius?
I bought the cable from local Toyota dealer and it doesn't match any of the 3 connectors on the back of the radio/tape player. I understand the two smaller connectors, one is supposed to plug in at the R4 slot and the other small connector plug plugs into R4. The larger connector does match the connector on the cd player. help, Dan
