Cracked Cyl. head, (in the combustion chamber) The crack will allow combustion gases (very high PSI) to pass, but not coolant (16 PSI) to pass. Very hard to find, even a compression test will not show this up.
Possible cylinder head water jacket or block crack or possible blown head gasket allowing compression into a water jacket.
You may not see signs of fuel in the coolant due to it being diesel and fuel is injected just before the piston is at top dead center, meaning uncontaminated air can be pushed into the cooling system before fuel is introduced into the system.
